Your parental feedback is invaluable in helping us improve the course and better understand its impact on students and families. Fully completing and submitting this survey is a mandatory step for your student to receive their BENERGY course payment. All questions must be answered.

Please answer each question based on your observations of your student throughout the course. And please be forthright. Criticism is not a bad thing. Your honest assessment, warts and all, is what will help us most.
